FORT THOMAS, KY (FOX19) -

Police released surveillance video and 911 calls related to the attempted murder-suicide in Ft. Thomas.

It happened just after 8 a.m. Tuesday at a medical office building at 20 Grand Avenue. The suspected gunman, DJ Mathis, is dead and Alisha Waters, his soon to be ex-wife, is in the hospital facing paralysis.

On Friday, surveillance video from the Florence gun shop 'Triggers' was released, showing Mathis purchasing the .380 handgun less than 24 hours before the shooting took place.

In the video, it takes Mathis about 30 minutes to pick out the gun and walk out with it.

DISPATCHER: 911, what's your emergency?

CALLER: Hurry, hurry. I'm at 20 North Grand Avenue. Someone's shooting downstairs.

DISPATCHER: Somebody is what?

CALLER: Somebody is shooting in our building downstairs.

Police responded immediately to the office buildings on North Grand Avenue and say that Waters, who works on the third floor of the office building as a receptionist for Personal-Touch Home Care, was attempting to get into the elevator when she was shot five times.

Rewind less than 24 hours from that scene to Mathis purchasing the handgun.
In surveillance video, he's seen testing it out before the purchase. A short time later, he walks over to another counter to fill out an application.

According to a statement released by Triggers, "Mr. Mathis completed all paperwork necessary to purchase a firearm… He was cleared by the FBI through their background check.  It was only after that FBI approval that the firearm was sold to Mr. Mathis."

About 25 minutes after he picked out the gun, he leaves the store.

FOX19 obtained a copy of the application, and it includes questions about mental illness or whether the gun purchaser was subject to a court restraining order, which you can read more about here: http://wxix.membercenter.worldnow.com/story/23078364/gun-used-in-attempted-murder-suicide-bought-day-before-shooting

While Mathis wasn't, FOX19's investigation revealed that Waters had tried to a restraining order, citing she was in fear for the safety of herself and her dogs. It was later revealed that because Waters never said Mathis physically abused her, the request was dismissed under Kentucky law.

Waters' family reported Thursday that she was communicating by blinking her eyes, and on Friday, they purchased an iPad to make communication easier.

Doctors say if she survives her injuries, she faces paralysis.
