Where does the athlete attend school?
What grade is the athlete in?
What is the student's GPA?
What sport(s) does the athlete participate in?
Name and phone number of athlete's coach.
What extra-curricular activites is the athlete involved in (school, community, service group, ect.)?
Why are you nominating this athlete?
